Anti-obesity oil -Is it a new innovation or rehashing of the old Japanese technology?

It is true that obesity is assuming epidemic proportions in countries like USA where almost one third of the population is either overweight or obese. Therefore any suggestion that can help to deal with highly dangerous global situation is worth getting a close attention provided it is logical, scientific and practical. What is tragic is that there is no unanimity about the exact cause of obesity though different experts opine differently defying any consensus. While consumption of saturated fats and trans fats, carbohydrates, white sugar and high fructose corn syrup are all implicated in one way or the other with obesity, the bitter truth is that humans are themselves responsible for this scourge visiting them through reckless dietary system combined with sedentary life style. There is unanimity that putting on body weight is directly linked to an imbalance between calorie intake and calorie expenditure and if the intake is more than what is necessary, even God cannot help in preventing increased body weight!. Though the theory is so simplistic to understand and to be acted upon by the people, the desire to eat good food ( purely from taste angle) over rides the need to keep the body weight constant for an adult. In other words, the will to moderate food consumption according to one's body needs seems to be lacking in case of most consumers resulting in over consumption. One of the curious facts is that healthy foods are invariably less tastier that nutrient light junk foods with lots of calories loaded in them and naturally such foods more or less create a sort of irresistible addiction, repeatedly attracting the hapless consumer!.

Billions of dollars are being made by the so called nutraceutical industry through peddling of hundreds of products claiming the magical properties of shedding body weight though their credentials are suspect. With food safety authorities choosing to look the other way without coming to the help of the citizen through appropriate control measures, the antiobesity products industry has a roaring time with their uncanny marketing strategy to trap vulnerable people like a "spider trapping its prey in its web"! In parallel with this development, another segment of the industry peddles "exercise" machines and crash "weight shedding courses" for the benefit of overweight people minting enormous fortunes. Who can forget the "famous" (or notorious?) Atkins diet or the South Beach diet and similar weight control regime peddling companies who were able to attract millions of people, scared about the ill consequences of high BMI which can cause a plethora of diseases including diabetes, hypertension, CVD, kidney ailments etc, crippling the life style even at an early stage of life. 

Latest to get the spot light is a romatic product going by the captivating name "antiobesity oil"! Though there are some traditional oil based concoctions in the market coming mostly from China and India, the new offering by the above name got a focused attention during the recent scientists conglomeration at Mysooru in the first week of January this year. According the flamboyant innovators, the antiobesity oil they just discovered could "maintain healthy body weight", provides "clean energy"  and "is not stored" as fat in the body. This product is claimed to have been developed for the "first time" in India from sunflower seed oil and it is "one of the kind in the world"! Added to this the antiobesity oil prepared at Mysooru is safety assured unlike similar ones developed elsewhere in the world, so goes the claim. According to the innovators this product is chemically identified as diacylglycerol oil which is supposed to be edible because the only difference between an edible oil as we are used to at present and their new product is that one of the three fatty acid molecules in the triglyceride oil products has been knocked off by controlled lipolysis. If the new product can be commercially made and approved by the food safety authorities in the country it can be a speciality edible oil for use by obese people to reduce their body weight. Of course the food industry needs to be convinced that the technology is "commercially workable" and economically viable. 

Let us look at the claim that such a product has been made for the first time in the world in India. Looking back one cannot forget that in early 2000 a product called "Enova oil" was developed and marketed in Japan claiming that less of this product is stored as visceral fat in the body unlike triglycerides which are the basic chemical entities of all edible oils. This product also boasted of unsaturated fatty acids which are known to reduce undesirable LDLs and raise desirable HDLs in the blood. However this product was abruptly recalled from the market because of several safety and health issues. One of them was the formation of glycidol during deacylation of the oil, an artifact implicated as potentially carcinogenic. Besides the ratio of Omega- 6 to Omega-3 fatty acids was 10:1 in Enova oil whereas most healthy vegetable oils have this ratio 3:1. High percentage of Omega-6 acids is reported to be associated with higher blood pressure and chronic inflammation. Thus the charmed marketing run of Enova oil abruptly ended in September 2009. Though the manufacturers promised to come back with the product soon, till to date there is no sign of this product, leading one to conclude that the technological and health problems were serious enough for the company to abandon it for ever.  

Another interesting fact which did not come to surface when the benefits of diacyl glycerol were glorified by the innovators pertains to its suitability for use as a frying fat. Most Indian preparations expose them to high temperatures between 160C and 210C and many conventional oils do cause frothing and foaming if the free fatty acid content is high in them. Also the mono glycerides of fatty acids are recognized as an excellent emulsifying agent in preparations where stable water in oil emulsions are to be made and in most counties their use in small quantities as emulsifier is approved as safe. According to FAO-WHO Alimentarius Commission daily intake of 12.5 to 25 mg of monoglycerides per kg body weight is considered safe. This works out to about 0.75 to 1.5 gm intake daily through all the foods consumed by an average person weighing 70 kg. Such data on diacyl glycerol are not readily available though it is generally assumed that the lipolytic enzymes present human body may be able to dispose of the product without much harmful effect. Against such a background if the new so called anti obesity oil is really going to be made for Indian consumers, adequate safety studies and tolerance limits will have to worked out before its clearance as a cooking oil..

The rabid"Fatophobia"-Consequences of past practices now being felt!

There has been a frenetic pace in the criticism of fats present in the diet during the last 3 decades, attributing practically every ailments at the doors of this food component present in all foods in varying quantities. The food industry world over has been castigated for manufacturing high fat containing and tasty foods that attract the consumers and dietary guidelines have been telling the consumers to shun foods rich in fat. It is true that fats, especially saturated versions, that too present in animal derived foods do cause obesity, diabetes, heart disease etc when consumed in excessive quantities though precise data were never generated as to how much is too much! American consumers and their counterparts in many affluent countries were told that fat consumption through the daily foods should not be more than 30% of the total calorie needs of the body. This did create a perception all around that fat is a "dirty" component in a healthy food to be shunned as much as possible. Even a school going child "knows" fat rich foods are not good for health! What are the consequences of such a great fat phobia? They are there to see in a country like the US where industry started pandering to this trend developing thousands of products with the so called "low fat" claims! 

How far we have been right in taking the fat phobia to such great heights? It now emerges that such commonly perceived beliefs were highly misplaced as reflected by the failure of such low fat foods to prevent the increase of obesity and other related diseases in these countries during the last 3 decades. It now emerges that the recommendation to cap fat calories at 30% of total calories was wrong and in stead of reducing the incidence of obesity, such a policy created the opposite effect! Why? Because the place of fat in the diet was taken over by carbohydrates which seem to have contributed to health afflictions. Of course the emphasis on unsaturated fats and plant derived fats was correct as they are relatively less harmful compared to saturated fats. Even blaming carbohydrates may not be wholly justified becauser the quality of carbohydrates consumed makes a big difference in deciding the healthiness of a diet. Food technology may have some thing to do with decreasing the carbohydrate quality progressively during the last 5 decades by developing newer technologies to "refine" food which effectively means removing healthy components during the processing.  

Fortunately a spate of recent research studies have been able to establish the futility of focusing too much on low-fat foods. Many recent scientific observations do confirm the above reality. Studies using large randomized trials involving thousands of subjects between 2006 and 2013 brought out the stark reality that a low-fat diet had no significant benefits in ameliorating incidences of heart disease, cerebral stroke, diabetes or cancer risks. In contrast a high-fat, Mediterranean-style diet rich in nuts or extra-virgin olive oil providing more than 40 percent of calories in total fat was able to significantly reduced cardiovascular disease, diabetes and long-term weight gain. There are also other similar studies which have shown that high-fat diets can have same effect or better than, low-fat diets for short-term weight loss. The types of foods, rather than fat content, relate to long-term weight gain. It is not proper to rule out completely the perception that high-fat diets are not healthy  or low-fat diets are less harmful. A universal principle that should be borne in mind is that too much focus on a particular food component in the diets can be misleading and food has to be viewed in a holistic way. Focus on total fat or other abstract numbers printed on the labels of packed foods will have to be replaced with right emphasis on eating more minimally processed fruits, nuts, vegetables, beans, fish, yogurt, vegetable oils and whole grains in stead of highly refined white grain flours, white potatoes, added sugars in what ever form and processed meats. 

Quantity of food eaten must be related to what we eat: As commonly believed cutting calories in the diet without improving food quality cannot be expected to cause long-term weight loss. It is this belated realization that has prompted health and food experts to modify the dietary guidelines, after singing the "song of 30% calories cap" for decades for consideration by the government without any upper limit on total fat. In addition, reduced-fat foods were specifically not recommended for obesity prevention. Instead, they want to encourage consumption according to healthful food-based diet patterns. The limit on total fat is an outdated concept, an obstacle to sensible change that promotes harmful low-fat foods, undermines efforts to limit refined grains and added sugars, and discourages the food industry from developing products higher in healthy fats. Fortunately, the people behind the Dietary Guidelines understand that but whether policy makers and the food industry take notice of this sane advice remains to be seen.

This discussion cannot be complete unless a mention is made about the life styles followed by the people to day,  especially with respect to their sedentary way of living with practically no exercise or any physical activity. The right way to lead a healthy life is to eat a balanced diet with adequate nutrition derived from whole grains, fruits, vegetables etc as mentioned above and do enough physical activity in any format that will burn the calories and achieve a dynamic metabolic activity in the body that can ensure sound health and mind.

Food gluttony that can destroy modern society -Why is man hurting himself through the food?

Food is a controversial subject being blamed for diseases caused by under eating as well as over eating. Hardly a day passes without any report blaming lack of food or imbalanced food for the under development of millions of people in the poverty ridden continents of Asia, Africa and South America. On the other hand same food is being implicated in causing a variety of modern day diseases among people in affluent countries, obviously due to over eating or consuming nutrition deficient diets. It is most unfortunate that food that sustains life on this planet is a cause for such a situation. It is not realized that food is like a weapon with multidimensional implications. If used wisely it can be a friend of human beings and if misused it can be a real threat to the lives of those who do not respect its virtues. Is it not a paradox that while some people struggle to get even the barest minimum of foods to survive there are others who indulge in gluttony and also waste foods unconscionably without a though for the poor? Is it not an irony that more money is spent to day across the world to fight health disorders and diseases caused by too much foods consumed than to eliminate the scourge of infection, malnutrition and under nutrition? 

We all grow up with this basic assumption dinned into our years that man needs a minimum of 2000 kC, 50 g of proteins, 50 g of fat and some micro nutrients essential which are not made in the body. Since last 3 decades dietary fiber and essential fatty acids were added as essential components of our diet. Interestingly these paradigm of health and nutrition remained almost static during the last hundred years except for some minor modifications. Has man changed much during this period? Of course the life styles of people who lived healthily hundred years ago were drastically different from the situation obtaining to day. The proportion of hard working people to the total population has been dramatically reduced during this period and the so called sedentary living is a rule to day than an exception! Why is that our great fore fathers produced children prolifically while to day two children family is the norm? Because they needed hands to help them to look after their avocation, be it agriculture or business. To day a farmer's son is unlikely to stay back in the farmland to carry on his father's tradition and culture and is more likely to end up in the urban ghettos that sprawl across hundreds of towns and cities across the country. Probably they want to earn fast and have no patience which is the most important trait of a farmer who has to fight with nature's aberrations and other uncertainties inherent in agriculture. 

If we look at the disease density in the urban areas and rural areas, though dependable statistics are hard to come by, many impartial observers agree that occurrence and prevalence of diseases like obesity, diabetes, hypertension in our rural hinterlands are relatively low. They are more vulnerable to water borne, air borne and food borne diseases due to acute safe water scarcity, utter lack of hygiene and less than healthy living environment. Ultimately it boils down to inadequate purchasing power and dilapidated infrastructure they suffer since decades with government not doing much to improve their lot. It may not be correct to blame the government alone for not doing many things which are within their ambit of action because lack of education and exposure to basic knowledge about good living also contributed to this Catch 22 situation. Hopefully this could change over the next few years if education facilities are expanded and employment opportunities are created. 

Why this talk about older generation and the present generation vis-a-vis food and health? During the last two decades thousands of studies have been undertaken to find solutions to modern day problems of health and invariably these studies focused on evolving methods and techniques to deal with diseases caused by over eating and wrong eating. Billions of dollars have been spent by several countries to deal with their citizens who have no control on the foods they eat and suffer from life style health disorders.While diseases like diabetes or blood pressure or heart disease are not curable easily but only to be managed through a plethora of drugs on which the pharma industry are making trillion of dollars churning out such drugs. There are two vital differences in the life styles of older generation population and the new generation in terms of physical work and quality of foods consumed.  Good old foods mostly unprocessed food grains, fresh fruits and vegetables, raw edible oils etc of yesteryears have yielded way to "unnatural foods" made by the food industry devoid of most of the nutrients including health protecting phytochemicals. As for physical work to day's generation has a living style not based on physical work but on intellectual work operating computers and mechanized gadgets in practically every walk of life. What is the necessity for such a person to have same calories, proteins and fats if they are not spent doing physical work? Naturally the excess energy accumulates in the body contributing to bloated bodies with expanded waistlines and consequent health related disorders. 

The restricted calorie diets and their favorable impact on life span has been brought out by a few studies pointing out why there is a need to cut down on calorie consumption, especially for those who lead a life with minimum body movement or exercise. To day's rush to develop remedies for the consequences of wrong eating or over eating is some what misplaced though as they may serve the limited purpose of medically dealing with those crossing the Rubicon knowingly endangering their lives. How ridiculous it is for perfectly normal people to pop in pills after leading an undisciplined eating regime to get relief for the ill effects of their indiscretion! Bariatric surgery and similar surgical intervention cannot be a routine procedure while many weight reducing medications should not have been there at all!  

Look at the latest "research" in this area  where some over enthusiastic scientists have come out with a modified chemical substance made from inulin, a natural oligofructosan found in Chicory, Jerusalem artichokes etc which is claimed to "kill" the appetite in human beings to a significant extent. On one side, highly attractive and tasty foods are created for the consumers to "enjoy" eating and then new chemicals are invented to curb their consumption! What a paradox! Both the food industry and the well being industry make money in this convenient alliance! Dietary discipline is what is needed to maintain the health and that is why dietary guidelines are formulated from time to time for the benefit of the consumers. Recent development of Inulin propionate esters (IPE), prepared from in the laboratory by reacting Inulin with propionic acid anhydride is being touted as a solution to curb the tendency for uncontrolled ingestion of food.       .  

In the case of IPE there is some rationale because Inulin is a soluble dietary fiber that does not contribute to any calorie, passing through the upper GI tract into the large intestine undigested. Obviously human body has no means of digesting this fiber but the microbiome inhabiting the gut feast on them producing short chain fatty acids considered beneficial to the health in many ways. What is so special about IPE? IPE is a combination of two naturally occurring substances inulin and propionic acid and therefore cannot be considered as a new chemical synthesized by scientists. Short chain fatty acids, (SCFA) which are organic acids with 1-6 carbons in their molecules have been known to exert influence on the metabolic system in the human body and among the most important ones are acetic acid, propionic acid and butyric acid which together make up 95%of the SCFAs in the gut. Propionates just make up about 25% while acetic acid happens to be the major SCFA. As propionates were found to exert a significant influence on excretion of gut hormones involved in satiety, it is often considered as a substance of therapeutic value. If so, is it possible to increase its concentration in the gut to a level that can exert significant influence on quantity of food eaten? This is where IPE comes into picture.

SCFAs are produced in the gut by microorganisms that reside there using residual food components not digested in the GI tract till they pass down to the large intestine. These include resistant starches, dietary fibers, sugar alcohols, undigested proteins etc but the concentration produced is not enough to exert any significant influence on satiety. If propionic acid levels in the large intestine can be increased, it can exert influence on gut hormone excretion that can achieve satiety fast. IPE can be made with different levels of esterification involving the hydroxy moieties in the fructose molecules of inulin. Higher the extent of esterification more effect it will have on satiety. IPE when ingested goes through the GI tract all the way to large intestine where microbes deesterify it to release propionic acid at levels that are effective in creating satiety and stop gluttony. If these findings are confirmed by other studies, IPE may become the darling of the well being industry and thousands of products incorporating IPE are likely to flood the market in the coming years. 

The question mark that why man is hurting himself through the food gluttony widely seen to day, may never be answered and no matter what remedies scientists come up with, the immediate provocation and seduction by good foods can never be resisted. As long as this continues, like cigarette and alcohol, food will cause lot of damage for those with fickle mind and irresistible cravings for junk foods! Sadly modern food industry, well being industry and pharma industry will be reaping the benefits from man's "Himalayan"  folly!

Calorie is not a calorie! How can that be?

It is universally admitted that too much sugar consumption can be harmful to health, especially in the long term but today's obsession with sugar gives a new twist to the controversy with people taking extreme positions to justify their views, without giving a chance to moderates for stating a balanced view. It is a famous idiom that "consumption of any thing in excess can become a poison, even if it is an elixir" and it applies to all foods, whether sugar rich, protein rich, carbohydrate rich or salt rich. Giving a new twist to the sugar controversy, in a recent publication an expert food professional has propounded a theory that "calorie in not a calorie" to highlight his view that calories derived from different foods can have varying effects on human body and cannot be expected to be equal in its impact on body. Can it be true? Let us examine.

According to this pundit, certain calories cause disease more than others. "Eat less, exercise more.' has been the gospel truth for reducing weight during the last 3-4 decades. Recall the history of American citizen's journey through the times during the last forty years and it does not require a brilliant brain to see that the policy makers have been consistently barking at the wrong tree attempting to improve the health of the citizens. First there was this euphoria about their supposed discovery that too much fat contributes to undesirable weight gain and distortion of the body. The industry willingly got into this band wagon turning out hundreds of products with low fat or no fat and interestingly the consumers trusted that these foods are good for their health. Imagine trillions of dollars raked in by the industry during the last 30-40 years exploiting the under current of fear among the consumers regarding the danger of becoming too fat! Then carbohydrate became the villain of peace and the industry grabbed this opportunity to promote low carb foods which were supposed to prevent weight gain. But no one bothered to check whether all carbohydrates are equally bad.

It is now that many pundits are opening their eyes and there is almost a near unanimity that natural foods as they are, cannot do any harm if consumed in moderate quantities. If any single player is to be blamed for to day's unenviable health situation mankind has boxed itself in, it is the food processing industry which for its own reasons created unhealthy foods through more and refining techniques, losing in the process vital nutrient components like dietary fiber, vitamins, minerals and many health protecting phytochemicals. In creating such a "Frankenstein", no doubt the food scientists, technologists and engineers aided the industry in developing processes and machinery that literally 'kill" the food and make it almost "toxic", to say the least! Testing the food for its nutritional value and health credentials cannot be done in isolated laboratories and values like PER, Digestibility, BV, etc can be only a rough guide for understanding the general characteristics of a food. These values for the same food can vary enormously among individuals and such variations according to ethnicity, region, environment, genetic history and many other variables can make significant difference in assessing foods and their health value. 

Coming back to sugar, the view that calorie derived by the body from white sugar is much more dangerous than that obtained from complex carbohydrates through its metabolic system, has some rationale behind it. The terrible obesity epidemic that confronts the US is not abating inspite of cutting down on fat, increasing exercise regimen and other recommended methods and in fact obesity rate seems to be increasing unabated. How an one explain a new born baby coming out as an obese one though it had neither the access to the so called bad foods or wherewithal to exercise while in its mother's womb? Naturally it pays for the sins of its mother who is obese herself! One of the most startling discoveries in recent years has been that sugar is an addictive substance and the human brain considers it as an addictive creating more and more cravings for the same like tobacco and alcohol products. Consumption of sugar products thus creates a vicious cycle of more and more eating whether the body needs it or not leading to uncontrolled weight gain.

The industry sensing an opportunity to rake in more money, knowingly or unknowingly, strains every nerve to develop more and more products containing more and more sugar and as the organoleptic qualities of these products are such that it is humanly impossible to resist consuming them as frequently as possible. For the last few years policy makers in many countries have realized the dangers of sugar, especially the added ones by the manufacturers, and have been advising the industry to cut down on added sugar to protect the health of the consumer. Unfortunately the action taken by the industry cannot be termed as responsible with many of its members continuing to indulge in sweetening their products excessively under the excuse that reducing sugar would affect the eating quality of these foods and of course their business too!. They use camouflaged terms like cane juice concentrate, brown rice syrup, fruit puree, corn syrup, high fructose syrup and many others to mislead the consumers about the presence of sugar in their products. Voluntary action can be effective only when all manufacturers collectively agree for a certain line of action which is not happening now. Probably mandatory restraint may have to be put in place to arrest this dangerous trend at national as well as international levels.

According to American Heart Association recommendations maximum levels of added sugar should not be above 24 g or 6 teaspoons per day for a woman, 36 g or 9 teaspoons a day for a man, 20-32 g or 6-8 teaspoons a day for children of 9 years and older and 12 g or 3 teaspoons for children between ages 4 and 8 years. If this is accepted imagine how much sugar is consumed through processed foods which are available in the market from a single serving?. For example yogurt is one of the most nutritious foods having very high probiotic properties and young children can considerably boost their immunity functions by consuming 1-2 cups of the same daily. But what did the industry do to this healthy product? Sugar, flavor and a host adjuncts have been added to create texture, color and texture of the product with the result that a single serving will supply about 7-15 g of sugar to a kid of 5 years old that is almost the entire day's recommended maximum! This kid will be ingesting more sugar through out the day when other food products like chocolates, biscuits, soft drinks, ice creams, pastry products etc and it is no wonder that such kids are right candidates for recruitment to the "obesity brigade" before reaching adulthood!   

Sugar's impact on the body is by now well recognized. It is known to alter the body's hormones and stimulate the need to consume more food. As with some recreational drugs, sugar spikes dopamine, a neurotransmitter that controls the brain's reward and pleasure centers and more demand is made, unconsciously, by the body to have more sugar with satiety not realized. Naturally one needs to consume more to get the same sugar high.every day. Sugar consumption, like alcoholism, is considered to be contributing to development of fatty liver, which in turn can cause liver failure, cirrhosis and ultimately liver cancer. At least this is the hypothesis that is being put forward by many health experts though clear cut scientific proof is yet to emerge. Nonetheless it is a possible risk and a warning that sugar must be avoided or restricted to the minimum. Body's energy needs can easily be met from other foods like food grains, plant oils and proteins which constitute the every day diet. Our palate enjoys sweet sensation and therefore this drives our desire to have more sweetened products to get the pleasure they provide in the oral cavity. Unless this urge is resisted with all the will power we have, sugar will continue to dominate our life leading us to our own extinction!

White Sugar vs Jaggery-indian contradictions

White sugar dominates the sweetener market in the country and both the sugar cane growers as well as the sugar mills have a symbiotic relationship and an organic linkage because of captive supply of cane grown in a certain area to nearby mills as per the law. Though India is one of the top sugar producing countries in the world, both the farmer as well as the industry are in doldrums due to many reasons. Payment arrears by the mills to the farmers run to thousands of crores of rupees while the mills are complaining about losses due to lower realization of prices for their product. Thus no one seems to be happy because of this multibillion industry as it exists to day. Adding to this problem is the declining prices ruling in the global sugar market making export also unattractive. If this is so, why not the government discourage cultivation of sugar cane and shift the land presently being used for cane cultivation for growing more critical food crops like oilseeds and pulses? Though this is an issue persisting for the last few years, no political courage seems to be evident among successive governments at Delhi.

There is another side story to sugar saga in this country which pertains to the low profile operation by a little known industry making another product from sugar cane viz Jaggery or Gud as it is known properly. Jaggery industry is a much neglected sector with no adequate oversight by the government, producing a range of jaggery products with non-uniform standards, filthy manufacturing environment and ill defined process parameters. Indian population uses jaggery as an essential ingredient in many of the traditional food preparations and the market can be enormously varying. Consumers are left at the mercy of the non-descript manufacturers and unreliable traders. Will this situation change for the better? After all almost 30% of the sugarcane harvested in the country estimated at about 260 million tons (MT) annually is thought to be going to the jaggery industry and this sector just cannot be ignored any more.

What is the difference between sugar and jaggery and why is that people prefer white sugar as a common sweetener in their day to day life? There are reasons such as the unattractive color of jaggery, its characteristic caramelized taste not liked by many, its unsuitably to make many products because of its physical properties, indifferent and non-uniform quality credentials and high cost compared to sugar. Jaggery is largely confined to micro and cottage scale operations near sugar growing areas and depend heavily on supplies of cane from the same belt as that linked to large sugar factories. Many traditional food preparations use Jaggery as a sweetener while sugar is the favored one for the food industry, especially for sweet products and by the house holds for beverages and traditional Indian sweets. What is intriguing is the vast price difference between sugar and jaggery, the latter almost 50-60% costlier though it is produced by a small scale sector with practically no over head expenditure in production costing. Besides jaggery yield is almost 100% of the sugar content in the cane juice making the process more economical. Management pundits may say that it is just a response to the demand supply situation!

Chemically white sugar is almost 100% sucrose with traces of minerals, Jaggery has about 3-10% water.with sucrose and reducing sugars being the major solids. Jaggery has some mineral constituents amounting to about 0.6 to 1%. Nutritionally the minerals like iron (10 mg%), Calcium (120 mg%), Magnesium (200 mg%), Potassium (800 mg%), Phosphorus (70 mg%) besides trace amounts of  Copper and Zinc. As it is a product made by prolonged boiling for hours together in open pans almost all biological nutrients are destroyed in the process. Still it is considered traditionally a nutritious substance used extensively in Ayurvedic medicines and traditional home remedies. No doubt white sugar production and over consumption in modern world is one of the great follies of mankind for which we are regretting to day collectively!

A typical Jaggey making unit is normally housed in a dilapidated tiled or asbestos roofed "shed". with no kutcha flooring, ill ventilated, full of cobwebs and fly infested and during the manufacturing operation the shed is ful of smoke literally choking the workers, doing the chores. The bagasse, after juice extraction is used as fuel for boiling the cane juice on an open hearth, naturally spewing out copious smoke all around. The cleanliness, sanitation and hygiene are so bade one has to see it to believe. Any consumer visiting such a unit will think twice before buying Jaggey again. Of course there are many exceptions where better managed Jaggery units also exist though they are far and few. It is in this context one has to appreciate the initiatives of the two agricultural university in Karnataka at Dharward and Bangalore deserve applause for setting up modern jaggery making units based on good manufacturing practices at Mudhol and Mandya. 

It is just about 2-3 years since these units were established and they seem to be doing a decent job considering the normal work pace associated with government institutions. The Jaggery Center at Mudhol in Bagalkot District is already reported to be producing organic Jaggery with a capacity to crush 40 tons of cane a day. The Jaggery Park as it is called was set up under the National Agriculture Development Scheme with an investment of about Rs 20 crore in 2011 and is supposed to improve Jaggery making process, infuse science in the practices, training of farmers, bring about cost effectiveness, preservation and packing improvements, minimizing chemical residue levels in the end product and further value addition to increase the farmer income. Besides solid Jaggery the center also makes Jaggery syrup and powder targeting different end users. Same applies to Mandya Jaggery Center also and both these units effectively meet the needs of more than 1000 organic cane growers in the state in selling their crops.  

One is really inspired by the achievement of the Mudhol Jaggey unit in creating a brand image for its products and exporting its products to countries like Russia within a short span of its launch. This jaggery according to the authorities there does not use unnecessary chemicals while manufacturing with only calcium carbonate, edible oil and wild okra going into its process. The Jaggery Park intends to promote setting up more such units as the entrepreneur can get a clean margin of Rs 900 per ton. According to the scientific team that is spearheading this mew initiative to promote jaggery, with an investment of Rs 1 crore a production unit to manufacture 40 tons of high quality organic jaggery, can be set up operated with the training provided by them. Probably more research is needed at food technology research institutions to diversify the use of jaggery in many products where sugar is extensively used at present.

Saturated fats in foods-Are they villains or just harmless players?

Thousands of treatise have been written, millions of words printed, umpteen number of seminars and workshops organized and many national and international advisories issued regarding the dangers inherent in consuming diets rich in saturated fats. Practically every school going child has been indoctrinated into believing that saturated fats are not good for health and almost every nutrition and health expert of some standing "villainize" these fats endlessly with no respite. Can we hold all of them irresponsible in making such claims during the last 3 decades? One cannot help getting a feeling that they are all prisoners of the big hype created against saturated fats based on inadequate interpretation and improper appreciation of the knowledge already available. It is good that this record of denunciation is being set right by recent massive studies to decide once for all whether saturated fats are really as bad as it has been made out to be.

According to new studies surfaced during the last 2-3 years saturated fat when consumed as a part of a balanced diet can never be harmful to the body though on a nutritional scale unsaturated fats may score over the former. Dramatic shift in the attitude of Westerners towards saturated fats in favor of their  unsaturated counterparts during the last 3 decades, did not bring down the incidences of diseases like CVD, Blood Pressure, Obesity and Diabetes among the population there! On the contrary this shift in dietary changes in favour of unsaturated fats containing MUFA and PUFA with emphasis on Omega-3-acids has increased the obesity and over weight population to a record high of 35% of the total population in a typical wealthy country like the US! Against this background can any one claim that consumption of more unsaturated fats has contributed to this epidemic? The lesson one has to draw is that diet needs to be considered in a holistic way, not based on the presence or absence of one particular ingredient such as saturated fat.  

One of the defining moments in the history of American dietary habits was demonizing saturated fats which led to a situation when industry literally went over board, curtailing and eliminating fats altogether from their products. Instead of fats, the new range of products contained more sugar and highly refined carbohydrates to replace the pared down fat ingredients. One of the consequences was that these products were not able to create necessary satiety in the consumer, to stop him from eating more, leading to the phenomenon of binge eating and uncontrolled weight gains. The industry did not stop at that and went on creating more and more eye catching products with high sensory attraction to tickle unsatiated consumers who responded to this by buying such products more frequently. It is said that a consumer can eat a product containing no fat almost double the quantity compared to a normal product with normal fat concentration! This is how the history of American obesity epidemic is going to be written for posterity. Can we claim that saturated fats caused this? Positively not! 

Health pundits are now more or less agreed that saturated fat when consumed from a mixed diet in which carbohydrate components are derived from whole cereals, unprocessed legumes, fruits and vegetables can never cause those diseases from which world is suffering to day. When discussing about fat,  one should not forget to mention the role of trans fats which are created by man in food products, by bad processing practices and which is really considered harmful to human body without any doubt. Where does it come from? Many creamy products to which people including kids are addicted, use hydrogenated fats made from liquid oils through catalytic saturation using heavy metal catalysts. It is rather mysterious why no country in the world has banned production of hydrogenated fats which during its process of manufacture generate trans fatty acids. The policy of restricting its presence to certain levels came rather late after some procrastination and mandatory declaration of the same on front of the pack labels are right steps that can contribute to better health of the consumers.

Though the above commentary makes it clear that consumers should not abhor saturated fats altogether, further work with human subjects through well designed clinical trials can confirm the above facts beyond a shadow of doubt. Mean while if one wants to continue to use liquid oils in preference to saturated fats like Palm oil or coconut oil they are free to do so, probably for a better health index. Recent publication about the beneficial effect of a blend of raw sesame oil and refined rice bran oil.on human beings, in terms of lowering cholesterol levels and reducing blood sugar levels is refreshing in these days of fat bashing! These findings clearly bring out that unrefined foods, as natural as possible can be the game changer for the survival of humanity with sound health.  

It is good that the world is slowly coming to the realization that low fat foods do not achieve much in terms of health and probably lower consumption of refined carbohydrates like white sugar, HFCS, refined grain flours and restricting salt intake would do more good in the long run. A low fat product loaded with refined carbs and salt may put the consumer in the comfort zone without realizing that such diets can be more harmful that saturated fats. Those advocates who promote low fat and zero fat consumption must realize that there are many fat soluble ingredients including vitamins, some micro nutrients and a variety of plant derived health protectants that will not get access to the blood stream unless adequate fat is present in the diet consumed every day. The ultimate "mantra" for good health is live like a normal human being as our ancestors lived by adopting a diet which is based on natural, unprocessed and unrefined food components as much as possible without caring whether they contain saturated fat or not!

The diabetic dilemma - A complex disorder with diverse dimensions

Is it not shocking when we are told that in this planet one human being dies every 6 seconds because of diabetes and complications arising from this scourge? The staggering figure of 384 million people being affected in his world by diabetes is indeed disturbing and look at the death rate-1.5 million (mn) dying because of this disease every year. How come this so called metabolic syndrome was allowed to creep in slowly without being taken seriously a few years ago? Probably adequate knowledge might not have been available then to understand unambiguously the basic reasons for human beings to develop diabetes. Progress in medical science and human physiology has created a fund of knowledge regarding various causes that can be responsible for spreading of metabolic syndrome in practically every country in this globe.

Saudi Arabia, UAE, Kuwait and Qatar are prominent countries where those affected by diabetes is as high as 20-24% of their population. That means 1 in 5 citizens suffers from this disease. According to the WHO the diabetic is expected to be prevalent among 600 million people by the year 2035. What about India? While as a percentage of the population it is small, about 5%, in absolute numbers, this country can be called the diabetic capital of the world. From 31.7 mn in the year 2000, the number of diabetics has jumped to 62 mn in 2014 which is predicted to reach almost 80 mn by 2030. India, China and the US have most diabetics compared to other countries though in terms of percentage figures, these countries have relatively less affected by diabetes measured per thousand population. Though statistics can be quite revealing, what is not understood is why some are more vulnerable while others are immune to this disorder, considered a life style disease caused by sedentary living conditions giving no scope for the body to expend the calories consumed through physical activities. 

If one asks the crucial question as to what is really responsible for this surge in diabetic population, the most common answer would be "obesity". According to present understanding about diabetes, over weight and obesity contribute to insulin insensitivity leading to a condition when body finds it difficult to push glucose into the cells where it is supposed to be converted to energy. However a closer look at the situation will reveal that obesity cannot be the sole factor causing diabetes. In the US there are almost 100 million obese people while the diabetic population is only 23 mn and it will reach a figure of 30 mn only by 2030. Similarly China the second largest obesity prone country has 46 mn cases of obesity whereas its diabetic population is only 30 mn. The third ranked India on obesity scale with 30 mn people suffering from obesity, has a whopping number of diabetics, viz 62 million. Thus it is difficult to establish a direct linkage between obesity and diabetes. There must be other reasons for developing diabetes which need to be looked into.

Three new studies carried out fairly recently have brought to surface the role of genetic make up, disturbed Amylin hormone cycle and disturbances in the body's natural circadian clock  Qualitatively some conclusions can be drawn regarding the role of genes and mutations undergone by them. The fact that certain ethnic groups are more prone to diabetes than others has been pointed out in support of genetic factors influencing the onset of diabetes. The inherent contradiction, that some lean people get diabetes while many over weight people are protected from diabetes, cannot be explained easily unless we fall back on genetic factors. A probable interpretation could be that the diabetes can occur with a 50:50 chance determined by family history and life styles followed by different people.

Those who contend that gene factor is important cite the 9 variations possible in the gene PPARG which has some influence on obesity development and which is not strictly related to BMI. Building up of fat around abdomen in some people who do not appear to be "fat" to look at, can be explained only though the above basis. Decreased function of PPARG can simulate such a condition. According to this school of thought diabetes that is seen in India and Pakistan is of this type. This is reflected by the fact that in India obesity is much less as a proportion to diabetic population than that found in the US and China. Diabetics to obese population in the US, China and India are 23:100:,30:40 and 62: 30 respectively reflecting the difference found in the above countries. 

The Amylin hormone produced by pancreas works in conjunction with insulin and plays a role in preventing over production or under utilization of insulin affecting glucose levels in the blood plasma. According to those who support the theory of Amylin hormone's role in developing diabetes, too much production of this hormone can get deposited around pancreas as toxic clumps causing death of insulin producing cells. This theory lacks precise scientific evidence regarding the cause of Amylin over production though it is thought to be related to inactivation of concerned enzymes involved in the metabolism of Amylin. But the qualitative finding as reported recently cannot be brushed away easily. Further studies can only throw more light on this aspect related to development of diabetes, at least among some people, not vulnerable to obesity.

The Circadian clock theory also appears rational though the evidence generated so far is with mice as subjects. The finding that during the night when mice are active, they showed high insulin efficiency to burn the foods they consumed into energy while in the same mice during day time when they sleep, their insulin resistance increases leading to accumulation of fat and weight gain. Insulin after all is  the driving tool for pushing glucose in the blood into liver, muscle and other cells. Mice population in which the gene responsible for circadian rhythm is knocked out were found to develop insulin resistance and consequently gained weight abnormally. Same thing was found when they were exposed to uninterrupted light for 24 hours upsetting their circadian rhythm. If such a phenomenon is confirmed by clinical trials with humans, it provides logical explanation for the vulnerability people working in night shifts and others ignoring the normal sleep cycle to diabetes more often. 

The above three findings throw lot of insight into diabetes epidemic the world is facing and must serve as a guide to future development for appropriate therapy to deal with this disease in the coming years. While diabetes related to genetic factors will have to be dealt with at the genetic level involving gene manipulating intervention drugs,  Amylin therapy to reduce insulin resistance will have to work at the enzyme level and circadian clock disruption will need to be avoided to pre empt this causing diabetes. The world is headed for an exciting era ahead when more precise and reliable drug therapy may emerge based on the latest findings. 

Beware, a person looking lean need not be free from diabetes while another person looking obese may be really non-diabetic!

Gene based dieting-Does it really work for body weight reduction?

Losing weight is a big business with many "experts" recommending special diets for achieving the objective of shedding body weight and bringing down body mass index (BMI) within the limits considered to be ideal. Millions of dollars are riding on this industry and in spite of all the efforts by scientists and the well being industry there is no guaranteed result oriented "treatment" or regime which can be considered successful to all the people. Who has not heard of Atkins Diet or Ornish Diet or South Beach Diet which had their golden run for a few years but eventually faded from the center scene. There are many health managing semi-medical type of centers, especially in India where special diets are administered to in-house patients for periods of one or two weeks. Though they are successful in rejuvenating the patients for some time, once they come out of the facilities and start on a regular diet the lost weight comes back with a vengeance. Is there no solution for this unenviable situation where man has found himself sucked into the obesity trap due to many reasons, most important being unbalanced dietary intake?

Food we eat to survive and lead a healthy disease free lifestyle has metamorphosed into a pleasure giving experience to day having lost the aura of health associated with it. Who is responsible for this distortion? Some say it is the industry which is "corrupting" the public with its reckless range of food products with progressively diluted nutritive value and full of food additives with doubtful safety credentials. Many sociologists feel that the food marketing strategy of the industry is to be faulted as large pack sizes and jumbo serving portions encourage consumers to eat or drink more than what they really need. Easy access to markets nearby and the personalized transport system invariably act as incentives for more eating. It is the stand of the nutritionists that most foods are designed improperly with sensory perception getting priority over nutrient content. Whatever it may be, fact still remains that over weight and obesity are here to stay with no sign of abatement in spite of many efforts at different levels. 

One of the most effective tools in the hands of the government is punitive taxation on fat-rich and sugar-rich products making these products more and more costlier. Such a policy is bound to show results in a short time but could be temporary as price sensitivity among people is relatively short lived. Tobacco industry still survives though smoking is becoming exorbitantly costly, especially for middle class population due to incidence of high taxes on them. Ultimately the responsibility for eating rightly and in a balanced way rest on the individual and unless there is a realization within that high fat and high sugar in foods can be "toxic" to their lives, very little can be achieved by the State. The argument that those who are obese need medical attention is another view increasingly being accepted but that cannot be a solution to a problem which is refusing to fade way easily. Assuming that the present obese population can be treated through drastic medical interventions, what about future? How are we going to arrest the growing trend of obesity and increasing number of people crossing the Rubicon? 

For quiet some time there has been a class of scientists attributing the tendency to put on body weight to genetic factors though no conclusive proof was forthcoming. The unsolved riddle of same diet causing one to put on weight while another one not showing this tendency could be the reason for the scientists to look at the genetic angle a little more carefully. The "brown fat vs white fat" issue brought out recently may be another angle to unravel the mystery associated with obesity. But it only helps to treat a patient by implanting stem cells that can differentiate into fat cells capable of making brown fats which are easily burned by the body. There is still another view which highlights the role of the human microbiome in deciding about the proclivity of any individual to accumulate body weight. The claim that some microorganisms can even be inherited from parents may be some what far fetched indicating that child of a fat mother must also be fat! 

It was in 2010 another twist was given to the genetic angle to obesity when a private company in the US and the reputed Stanford University collaborated and brought out a starling fact that disposition to put on weight is encoded in the genes of individuals. They were able to decode the variations in some genes that make a person vulnerable to illogical weight again. The science of Nutrigenomics is a relatively new discipline and linking of diet to genes is opening up new avenues to treat obesity. Certain genes like ABP2, ADRB2, PPAR-gamma are shown to have variations in their single Nucleotide Polymorphism (SNP) which can predict whether a particular diet can cause overweight. The FTO gene which contributes to Fat mass and obesity related protein is reported to have high impact on body weight. According to this theory variations in FTO gene can cause an increase in the levels of the hunger hormone Ghrelin which makes dieting extremely difficult for such an individual. Those with FTO gene variations may have an irresistible craving for foods with high fat and high sugar.

How can all the above "revelations" help in tackling obesity? If some of the well being industry players are to be believed all it needs is to take a swab from the cheek of a person to finger print the DNA make up and based on this, right type of diet can be prescribed for him. Thus people with the DNA type that can favor low carbohydrate diet must go for the same without bothering too much about the fat content. to get the best result. For others low fat diet may be more appropriate.to achieve the goal. In India one of the fitness specialized companies is offering the so called  "DNA FIT" program claimed to be "a custom-fit solution" that can "answer for all those who have been seeking a scientific, individualized weight loss program that offers speedy, yet sustainable results". It also claims to be Asia's first DNA based weight loss program. How far these claims are realistic cannot be verified in the absence independent opinion from experts, though it offers some hope for those desperate to slim down after failing all other routes to achieve positive results.

A million dollar question that still remains unanswered is how come there is such a large gene variation happening just in the last 10-15 years when obesity rate started climbing steeply and showing no signs of abatement. Is this due to the environment which is precipitating this epidemic to cause such widespread changes in the DNA make up of people? Or could it be due to increased consumption of processed foods containing many additives having this effect on the DNA? What ever it is, how can the world manage to reverse this trend with almost 16% of the population vulnerable to obesity and 70% of the population with 50% chances to become obese based on their gene make up, unless they change their eating habits according to their DNA type? It is paradoxical that ultimately the focus comes back to individuals who have to take action as per the above findings and DNA picture provides only a suggested solution for adoption.

GUT BACTERIA-WILL THEY HELP ATTACK OBESITY EPIDEMIC?

How desperate people can be when all measures to control body weight come to naught and drastic approaches like bariatric surgery become the most practical step. However such strategies cannot be a standard solution to the problems of millions of people who are confronted by the weight gain syndrome. It is true that the basic scientific foundation for explaining body weight gain is the excess intake of calories through carbohydrates and fats in the diet that become fat deposits in different parts of the body causing weight gain. Balancing of the diet with regard to calories is a prerequisite for avoiding weight gain.

The million dollar question is how to balance the diet vis-a-vis calories? Unless one is aware of the calorie expenditure through physical activity it is next to impossible to decide what and how much to eat. Though a 2000 calories diet is recommended for a healthy normal human, it can be insufficient if the physical activity is too high as encountered by athletes and others doing heavy work. On the other hand a sedentary person with minimum physical activity does not need that much calories through the daily diet. While obesity is an ugly disease with the person suffering from it presenting a grotesque picture and bloated body cynosure of all eyes in public places! While physical appearance is one undesirable feature of obesity, much more crucial is the vulnerability of these unfortunate victims to serious diseases like heart attack, stroke, diabetes, kidney disorders etc.

The Body Mass Index (BMI) is supposed to tell whether one is obese or not and generally any figure beyond 30 is considered dangerous while BMI in the range 20-25 is desirable. There can be many contradictions in putting obesity on a BMI scale with some persons having normal health even when BMI exceeds 25 while some low BMI individuals do suffer from many diseases like diabetes and heart ailments, obviously due to reasons other than high body weight. Though there is a general belief that food intake and exercise can minimize the chances of obesity there can be other reasons for individuals to get high BMI in spite of low calorie diet and regular exercise. Unfortunately the current understanding of this area of human science is not complete and with each passing day new information is emerging regarding the various factors that play different roles in causing obesity.

It was quite recently that scientists discovered about the role of gut bacteria in human health and hundreds of studies have brought about the fact that bacteria in general and intestinal bacteria in particular play an important role in maintaining good health. The fact that new born babies with practically a sterile intestine acquire rapidly a host of bacteria through mother's milk indicating that even different bacteria residing in different parts of the body of a human being play decisive roles in protecting the person as a natural defense and use of chemicals and other antibacterial materials used liberally for keeping away disease causing pathogens can be, in reality, a foolish thing to do in the long run! No one knows precisely how much damage such sanitizing agents can cause to disturb the equilibrium that exists among different species of bacteria that colonize human body.

"Fecal transplant" which has become a standard treatment regime for patients suffering from incurable diseases like Crohn's diseas, Irritable bowel syndrome and others is a telling commentary on the usefulness of bacteria in providing relief to human beings from serious diseases. Though it may sound repulsive to think about transferring feces from one person to another one,  the success of this approach has been so dramatic that thousands of people are becoming candidates for fecal transplant every year all over the world. What is not clearly understood is the identity of the particular species or combinations of them that is providing the relief to the recipients and how long it will take to come out with an answer is any body's guess. But dramatic break through can be expected in the foreseeable future in this critical area.                      

Recent studies focusing on the likely role of gut bacteria on development of obesity are no less dramatic in unfolding another dimension to the beneficiary role played by bugs. in what is considered a solid evidence researchers used pairs of human twins in which one was obese and the other lean. On transfer of gut bacteria from these twins into mice revealed some startling facts which may have long term implications on obesity research. The mice with bacteria from fat twins grew fat while those that received bacteria from lean twins stayed lean! (those that got bacteria from lean twins stayed lean). It is just an empirical observation which needs to be interpreted carefully without drawing any definitive conclusion. With these studies it does not become logical for obese people to become right candidates for fecal transplant. Unless scientists are able to isolate most important bacterial species present in the feces which are responsible for the effect, the scientific theory behind such treatments cannot be unraveled.

What is not to be forgotten is that human biology is so complex it is difficult to pin point what all factors contribute to the phenomenon of obesity. These include genetics, diets and living styles followed by different people. Interestingly it has been shown that with the right diet it should be possible to  change the bacteria in a fat person's gut so that they promote leanness rather than obesity. Further in the presence of a low-fat diet, bacteria from a lean twin can take over the gut of a mouse that already had bacteria from a fat twin causing significant weight loss in the latter but the opposite does not happen and no matter what the diet, bacteria from a fat mouse do not take over in a mouse that is thin. It really adds up to a quiet riddle which needs some unraveling by the scientific community.
